Questionnaire (This paper version of the questionnaire does not match exactly the latest computerized version of the questionnaire, although differences are minimal.)

The PREHCO Questionnaire has different versions depending on: the existence of a spouse and the capability of target and spouse to answer the questionnaire by themselves.
